1. Completed 2023 Spring Challenge Player Liability & Parent Conduct Form
2. Official State Certified Tournament Roster OR Official State Certified Team Roster OR Official State Certified Travel Roster
3. Permission to Travel Documents (Out of State Teams Only) **State Approved
4. Guest Player Roster (If Applicable) **5 Max
5. Player card copy of all team members. Include Coaches and Players (NOT required for teams playing in the Recreation Bracket).
6. Mark through Players on Roster not participating.

​Player card for each player, guest player(s) and coach(es) will be checked at the fields prior to each game. Only Laminated player cards or Official Digital Player Card System will be accepted.
