The application only needs to be completed the first soccer season the player joins a club associated with US Youth Soccer or US Club (not yearly). The application and supporting documents are used by USSF to receive permission from the soccer federation of the player's country of birth for the player to participate in a USYSA or US Club sanctioned club.
Players WILL NOT be allowed to participate in games or tournaments with their assigned team while the USSF minors international clearance process is pending, for this reason we ask that you, the parent, turn in the required application, forms and supporting documents to the CESA office as soon as possible!

Which application/ form does my child need?
1) Is the player already 18 years old and was born outside of the United States?
If the answer is yes, complete and sign the "Adult First Registration (link)" application and return it to the CESA office along with a pdf copy of the requested supporting document.
2) Is the player a United States Citizen born abroad and has never played soccer for a soccer group, soccer club or soccer organization in another country?
If the answer is yes, complete and sign the "Minor_FR Citizen (link)" application and return it to the CESA office along with a pdf copy of the requested supporting document.
If the player is a US Citizen born abroad that has played soccer in another country complete the "Parents Move Exception" application.
3) Was your child born in the years 2016, 2017, 2018, 2019 or 2020? Or was your child born in the year 2015 AND is currently 9 years old?
If the answer is yes, complete and sign the “Minors P10 (link)” application. Return to the CESA office along with a pdf copy of the player’s original birth certificate or Passport.
4) Was your child born between 2007 through 2015 (Currently between 10 and 17 years old) AND has the child attended school in the United States since at least the Fall 2019 or earlier?
If the answer is yes, complete and sign the “Minors Clearance Application 5 Year Exception (link)” application and return to the CESA office along with pdf copies of ALL the requested supporting documents.
5) Was your child born between 2007 through 2015 (Currently between 10 and 17 years old) AND has the child lived in the United States less than 5 full consecutive years?
If the answer is yes, complete and sign the “Parents Move Exception (link)” application and return to the CESA office along with the completed “Parents/Legal Guardian Statement (link)” form and pdf copies of ALL the requested supporting documents.
6) Was your child born between 2007 through 2015 (Currently between 10 and 17 years old) AND has moved to the United States due to humanitarian reasons such as seeking refugee status and cannot be expected to return to their country of origin?
If the answer is yes, complete and sign the “Humanitarian Exception (link)” application and return it to the CESA office along with pdf copies of ALL the requested supporting documents.
